InnovAge PACE can be an excellent option for individuals with limited financial resources or declining ability to perform activities of daily living. The program provides coordinated personal and skilled care in the home, as well as in the Day/Health Centers where socialization, meals and recreation are also available. A care plan is created for each program participant by an interdisciplinary team of professionals. Based on identified needs, the team works with the participant and their family to create a plan of care and outlines the services they will receive. Participants are treated and checked regularly, and the care plan updated as needed.
Tours of the Center are available after the presentation to see the PACE program in action.
